05 Outliers and System Actions

Who gets flagged

  • Providers: score below 70% with at least 5 charts. The volume floor keeps small samples off the list.
  • Facilities: below 70%, or declining by 3+ points even if the level is fine.

The 70% threshold is stricter than the 60% red band, so an amber 65% provider is still an outlier. That is expected.

The three cards

  • Provider Outliers: worst first, with score, volume, change and tags (Declining, Improving, Rank n/m). Click a row for the trend view.
  • Facility Outliers: tags Below 70%, Declining, or Recovering (up 2+ points). Recovering on a below-threshold site is evidence an intervention is working.
  • System-Wide Actions: cohort-level wins and risks with a priority and a suggested action. These are conversation starters for your ops meeting, not buttons.

The trend drill-down

Opens from any outlier row, or any row in the section 03 and 04 band lists. It always covers the last 90 days, regardless of the time pills.

  • Stat strip: charts, peer rank (providers) or provider count (facilities), volatility, range, and momentum for facilities.
  • Weekly bars with control lines: the middle line is the average weekly score; the upper and lower dashed lines sit two standard deviations away. A bar outside the lines is a statistically unusual week worth investigating. Movement between the lines is ordinary noise.
  • Indicator table: every indicator for that provider or facility, filterable by chart section and sortable. Each row has a Goal button.
  • Summary sentence: direction, change, threshold advice and the biggest-moving indicator. Calculated from the same data, so it is safe to paste into an email.

A coaching workflow

  1. Open your lowest provider outlier.
  2. Filter the indicator table to its worst section.
  3. Note the biggest mover.

You now have a name, a section and an indicator: a complete coaching conversation.
